- Enrollment Interest Application / Enrollment Interest Application (Spanish) - this form is to be filled out if you are interested in enrolling your sudents at NSLA. Email the form back to Enroll-NSLA@lcer.org
- Update of Contact Information & Emergency Pick Up Person - this form is to be filled out whenever a change of address or telephone number occurs. Also you can add emergency pick up person to your childs emergency card. It is critical to keep updated information on file with the school.
- Student Withdrawal/Exit Form - this form is for students who are leaving our school for whatever reason. It helps the staff to watch for trends that might be necessary to address as a staff.
- Student Uniform Order Form - This order form is to be filled out and turned into our PTO President for tshirt uniform order. Or you can follow the link for the online store: https://spirit-wear-store-13114.cheddarup.com
- Dental Assessment Form - this form is for new kindergarten students. This form must be submitted to the registrar prior to the start of school.
- Kindergarten Health Examination Form - this form is for new kindergarten students. A health exam must be completed prior to starting school.
- Medication Form (Physician's Instructions Form, in SPANISH) - this form is for students who require the administration or the possession of medicine and is to filled out by the student's physician and parent/guardian.
- Photo Release Form - this form is for parents who do not wish their child's picture or information released for publication. Please fill it out and drop it off with our registrar.
